For your convenience, the Office of Vital Statistics (birth certificates) in conjunction with St. Joseph Women’s Hospital has opened a satellite office on the first floor of the St. Joseph's just behind the guard station.
Issuance of birth certificates is available between the hours of 8am to 4pm Monday through Friday.

commonly asked Vital Statistics questions.

NEED A BIRTH,
DEATH, MARRIAGE OR DIVORCE CERTIFICATE FROM ANOTHER STATE?
The CDC has provided a web site for
those who want direct access to individual State and territory
information. To use this valuable tool, you must first determine
the State or area where the birth, death, marriage, or divorce
occurred, then click on that State or area.

FOREIGN BORN AMERICAN BIRTH AND DEATH RECORD INFORMATION
Persons in need of a copy of a foreign birth or death record should contact the Embassy or the nearest Consulate in the U.S. of the country in which the birth or death occurred. Addresses and telephone numbers for these offices are listed in the U.S. Department of State Publication 7846, Foreign Consular offices in the United States.

Commemorative
Birth and Marriage Certificates

The State of Florida
now issues commemorative birth and marriage certificates, signed
by the Governor and State Registrar of Vital Statistics.
The certificates contain calligraphy style printing, gold embossed
state seals, and are suitable for framing and preserving as
family heirlooms. The commemorative certificates are mailed
encased in cardboard shields to ensure protection.
