If I come to your office during your hours of operation, can I receive my Birth Certificate right away?
In most cases, yes.

What is your usual waiting time if I come in person?
It takes approximately 30-40 minutes.

What is your usual waiting time if I mail in my request?
From 10 - 12 working days.

Can I order a birth or death certificate by phone, fax or online and have the certificate sent United Parcel Service (UPS)?
Yes, we have a private firm that accepts all major credit cards.

What do I have to include when mailing in for a birth certificate?
You can print the form or enclose a letter stating the Registrants name, date of birth, county of birth, mother's maiden name and father's name. Include a copy of your identification, preferable a State issued license or identification card. Birth Certificates are $14.00 for the first copy and $9.00 for each additional copy of the same certificate purchased at the same time. We accept only certified check or money orders.

Do I need proper identification?
Definitely, preferably a State driver's license, as Birth Certificates are confidential in the State of Florida.

What years do you have available for Birth Certificates?
Presently, we have birth certificates for the years 1930 to the present for anyone born in the State of Florida.

If I was born in Florida before 1930, how do I get my birth certificate?
Please write to our main office, or call them for more information.

How do I go about correcting or amending a birth certificate?
If you need to amend a spelling error, you can call us and we will send you the amendment papers through the mail.

How do I get the child's father's name on the birth certificate?
If you were not married to anyone else at the time of birth, we can send you the paternity papers through the mail. You and the father can fill out the paperwork and forward to the main office (address is stated on the form).

What if I was born in another state?
Call us and we will give you that address

What if I was born in another country?

What is your turn around time on mail requests?
Up to 14 working days

Do I have to have the birth application notarized before mailing to your office?
You can send us a copy of your State Driver's license or State Identification Card or you can have the form notarized.

You can personally apply for a birth certificate at any one of our outlying satellites, but you will not receive it the same day. Birth certificates applied for at any of our clinics will be mailed directly to your home.

Are there any restrictions on who can receive a Birth Certificate?

ELIGIBILITY:
Certificates can be issued only to the registrant (the child named on the record) if of legal age, (18) parent, guardian, or legal representative or one of those persons or by court order. In the case of a deceased registrant, upon receipt of the death certificate or the decedent, a certificate of the birth certificate can be issued to the spouse, child, grandchild, sibling, if of legal age, or to the legal representative of any of these persons as well as to the parent.

Are there any restrictions on who can receive a Death Certificate?

ELIGIBILITY:
Death records with the cause of death information may only be issued to the following individuals: the decedent's spouse or parent; to the decedent's child, grandchild or sibling, if of legal age; to any person who provides a will, insurance policy or other document that demonstrates his or her interest in the estate of the decedent, or to any person who provides documentation that he or she is acting on behalf of any of the above named persons, All requests for certification of a death certificate that includes the cause of death information must include signature of the applicant, state his or her qualifying eligibility, or a notarized Affidavit to Release Cause of Death Information (DH Form 1959), which is available upon request.

Presently we have Hillsborough County death certificates for the years 2005 to present.

What information do I need to get a death certificate?
You can print the form or enclose a letter stating the decedent's name, date of death, your relationship to the decedent. If you require the cause of death, please enclose a copy of your identification. Death certificates are $10.00 each. We do not accept personal checks.
