CHILDHOOD IMMUNIZATIONS
All health department clinics provide vaccines for children who
do not have insurance that covers immunizations are free of charge.
Children under 18 years of age must be accompanied by parent or
legal guardian. In an emergency where the parent/guardian is unable
to accompany the child, a grandparent, adult sibling, aunt or
uncle or stepparent may sign for the child, providing they have
knowledge
of the child's past medical history and any allergies or problems
with previous vaccinations. Any other adult accompanying the child
must provide a notarized note from the parent or legal guardian
indicating that the named individual accompanying the child has
their permission to authorize medical treatment for their child.
Immunization records must be presented at the time of service.
Parents should plan to bring a copy of their child's record on
each visit to ensure they have an up to date record at all times.
Children must have an immunization record on file or a copy of
their record from the school or their private physician in order
to receive the proper immunization.
The State of Florida form 680 ("Blue Card") is available
at all of our clinics. This form is required for daycare and school
entry. Children must be up to date with immunizations in order
to receive this form.
"AFTER THE SHOT-----" - There is always a slight chance
of side effects after an immunization. Most are mild, like a slight
fever or sore arm. Pain and fever relievers are good to have on

In-School immunization
clinics - your health department provides immunizations for children
at school. For a list of the currently scheduled in-school clinics,
check our "School Outreach" section on the Immunization
Home Page.
If your child's school doesn't appear, ask the school nurse to
consider an in-school clinic.

NEW LAW - Beginning January 1, 2003, all students
residing in on-campus housing must provide documentation of the
Hepatitis B series, 2 MMRs and a Meningitis vaccine. Hepatitis
B and MMR are provided as part of the childhood series for 18
and under. Meningitis vaccine is a one time shot at a fee of $72.00.
